Our Process
- Care assessment - NDC provides you with a complimentary care assessment, available either in-home, where we thoroughly discuss your unique care requirements, direct care worker preferences, and any special considerations. This ensures that we are in sync with the details and expectations of your personalized care plan.
- Care Plan - the care assessment is turned into your own personal care plan which includes preferred days and times of service, the sequence of services needed on those days, additional advice we may provide based on our industry experience.
- Patient Administration - the care plan is formalized and service can begin once we complete a client administration form with your sign off and confirmation of the first day of service.
- Start Service - we completely understand the initial unease of entrusting your beloved family member to a direct care worker who may initially feel like a stranger. Please take comfort in knowing that you'll soon come to see your NDC direct care worker as another cherished member of your family, filled with love and compassion!
Our Care Givers
There are three key components to any NDC direct care workeras we pride ourselves on the consistency and leadership within our care team.
Training – every NDC direct care worker is rigorously trained through onboarding and periodically throughout the year. We have a robust training program, meant to ensure our direct care workers provide the best differentiated care in the industry.
Certification – NDC direct care workers are PA disability and mental disability waiver program certified to help you with any disability concerns. Call us today to learn more about PA disability and if you qualify.
